Telehealth Consent

Effective date: July 7, 2026

1. Your Agreement to This Consent

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911. IF YOU ARE IN CRISIS, CALL 911 OR THE 988 SUICIDE & CRISIS LIFELINE AT 988.

BY CREATING AN ACCOUNT, COMPLETING AN INTAKE, OR OTHERWISE USING TELEHEALTH SERVICES ACCESSED THROUGH HEALTH ROCKIT, YOU CONFIRM THAT YOU HAVE READ AND UNDERSTOOD THIS TELEHEALTH CONSENT AND THAT YOU VOLUNTARILY CONSENT TO RECEIVE HEALTHCARE SERVICES THROUGH TELEHEALTH. IF YOU DO NOT AGREE, DO NOT USE THE SERVICE.

This consent should be read together with our Terms of Service and Privacy Policy.

2. Purpose

The purpose of this document is to obtain your informed consent to receive telehealth services from licensed physicians, physician associates, and nurse practitioners (collectively, "Providers") accessed through the Health Rockit platform. It explains what telehealth is, its expected benefits, its known risks and limitations, the alternatives available to you, and your rights as a patient.

3. Use of Telehealth

Telehealth is the delivery of healthcare services using electronic communications between a patient and a Provider who are not in the same physical location. Depending on your state and your clinical circumstances, your care may involve one or more of the following modalities:

  • Store-and-forward (asynchronous) review — a Provider reviews the health history, questionnaire responses, and other information you submit through your secure online intake.
  • Secure messaging — written communication with your Provider or care team through the patient portal.
  • Audio or audio/video visits — a live (synchronous) consultation, which some states require before a prescription may be issued.

Your Provider will determine which modality is clinically appropriate and consistent with the laws of your state. Providers deliver care only in states where they hold an active license.

Alternatives. In-person services are an alternative to telehealth, and you may choose an alternative at any time. Nothing about this consent prevents you from seeking care from an in-person clinician, urgent care clinic, or hospital, and your Provider may direct you to in-person care whenever it is clinically indicated.

4. Anticipated Benefits

  • Access to medical evaluation and, when medically appropriate, treatment from your home or another private location, without travel or time away from work.
  • Review of your health information by a licensed Provider in your state, on your schedule.
  • Continuity of care through secure follow-up messaging with your care team.

No particular benefit, result, or outcome can be guaranteed from telehealth or from any treatment.

5. Potential Risks

As with any healthcare service, telehealth carries risks. These include, but are not limited to:

  • Information transmitted electronically may be insufficient in quality or completeness to allow appropriate medical decision-making (for example, unclear responses or missing records).
  • Technology failures, interruptions, or delays may disrupt or delay evaluation, communication, or treatment.
  • The absence of an in-person physical examination, vital signs, or laboratory testing may limit your Provider's ability to diagnose or treat certain conditions.
  • In rare circumstances, security protocols could fail, creating a risk of a breach of the privacy of your health information.
  • State laws may limit what a Provider may prescribe through telehealth, and some medications cannot be prescribed without an in-person visit.
  • If you do not provide complete and accurate information about your health, medications, and allergies, there is a risk of adverse drug interactions, allergic reactions, or other errors.

You may not receive diagnosis or treatment for every condition through telehealth. Your Provider may determine that telehealth is not appropriate for some or all of your needs and may direct you to in-person care.

6. Emergency Situations; Follow-Up Care

Telehealth services accessed through Health Rockit are not designed for, and must not be used for, medical emergencies or urgent conditions. Providers are not available on an emergency or on-call basis. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. If you are in crisis, call or text 988 (Suicide & Crisis Lifeline).

If you experience side effects, a worsening condition, or new symptoms after beginning treatment, message your care team through the secure patient portal and follow your Provider's instructions. Your Provider may recommend follow-up telehealth visits, laboratory work, or in-person evaluation as part of your ongoing care.

7. Data Privacy and Protection

The laws that protect the privacy and confidentiality of medical information apply to telehealth. Your health information is transmitted and stored using electronic safeguards designed to protect it, and it is used and disclosed as described in our Privacy Policy, which explains what information is collected, how it is used, and the rights you have with respect to it. Please review it carefully.

8. State Licensure

Telehealth services are available only in states where participating Providers hold active licenses. Health Rockit does not currently serve California, South Carolina, or New Jersey, and availability may change based on provider licensure and regulatory requirements. You agree to provide your true physical location at the time of each consultation, as the laws of that state govern the care you receive.

9. Your Acknowledgments and Rights

By consenting to telehealth, you acknowledge and agree that:

  • No treatment or prescription is guaranteed. A Provider will issue a prescription only when, in the Provider's independent clinical judgment, treatment is medically appropriate for you.
  • You have the right to withhold or withdraw this consent at any time, in writing, without affecting your right to future care or treatment. Withdrawing consent ends telehealth services but does not apply retroactively to care already provided.
  • You must provide truthful, accurate, and complete information about your identity, location, health history, medications, and allergies, and update it if it changes.
  • Telehealth sessions are not recorded by the Provider or by Health Rockit.
  • You are free to obtain your prescription from any pharmacy of your choice by contacting support and notifying your prescribing healthcare professional.
  • The Service is cash-pay. You must pay the full amount of costs associated with use of the Service, including prescriptions, and will not submit claims to Medicare or other insurers.
  • You may ask your Provider questions about telehealth, your evaluation, or any recommended treatment at any time, and you may decline any recommended treatment.
  • If you have a concern or complaint about the care you received that cannot be resolved with your Provider or our support team, you may contact the medical board of the state in which you received care.

10. Who Provides Your Care

Health Rockit does not provide medical consultations. Consultations are provided through LocumTele, an independent network of licensed providers.

This Telehealth Consent is effective as of July 7, 2026. Every plan is reviewed by a licensed provider before any prescription is issued, and prescriptions are issued only when medically appropriate.
